At The Plaza at Clover Lake, our goal is to encourage our residents to maintain an active and independent lifestyle while getting the assistance they need in a warm and loving atmosphere that each resident can call home.

At The Plaza at Clover Lake we partner with each resident to design an individualized plan of care, which is based off of each resident’s nursing assessment, to provide quality nursing services and assistance with activities of daily living. Our dedicated and trained staff are available to care for each resident 24 hours a day, 7 days a week, ensuring that each resident feels comfortable and secure in his or her home. We coordinate with various health care providers to deliver a continuum of care and comprehensive service to you or your loved ones.

The Plaza at Clover Lake’s Assisted Living Program (ALP) provides eligible individuals with New York State Medicaid-funded personal care services. Certified Home Health Aides provide assistance with personal care, under the direction of our Nursing team, ensuring a dignified and comfortable experience catered to our Residents’ individual plans of care.
